What we do

The records we retrieve and the work around them

GP and primary care records

We approach surgeries across the UK for full patient histories, and we follow each practice's own process for consent and payment rather than assuming one size fits all.

Hospital and trust records

We request inpatient, outpatient and accident and emergency notes from NHS trusts and private hospitals, keeping track of each department that holds a part of the file.

Mental health and specialist records

We handle requests to mental health services and specialist departments, where responses often need more careful chasing and a clearer explanation of why the records are needed.

Imaging and scan reports

Where radiology reports, scan images or discs form part of the picture, we request them alongside the written records so the file arrives in one delivery.

Ongoing chasing and status updates

We keep chasing providers who have gone quiet, and we keep you informed of where each request has got to, so you are never left guessing about progress.

We do not provide medical opinion, expert witness reports or any interpretation of what the records contain, as that sits with your own medical experts.
Requests that arrive without valid patient consent or a lawful basis for disclosure are declined, because we will not approach a provider without the paperwork in order.
We do not take on records held outside the UK, since our knowledge of how providers work stops at the British systems we deal with daily.
Where a provider has already refused disclosure and there is no route to appeal, we will tell you plainly rather than keep billing for chasing that cannot go anywhere.

Process

How a request moves from instruction to file

We check consent and paperwork

Before we approach anyone, we make sure the consent form, identification and any authority to act are present and correct, since providers will turn a request away without them.

We request and chase the records

Requests go out to each provider, and we follow them up on a schedule of our own, dealing with queries and payment requests as they come back to us.

We collate and hand over the file

Once everything has arrived, we check it against the original request, put it into order and send the completed bundle to you with a note of anything still outstanding.
