Process
Tell us the practice or hospital, the dates of care and your signed consent form, and we'll confirm the scope of what you need.
We write to the records manager with your authority attached, asking for the complete file and logging the date we made contact.
If the practice is slow, we follow up until they release the records, then arrange secure collection from their premises.
The file is scanned, checked and indexed, and you receive a digital copy or a printed set by the method you've chosen.
Questions
It varies. Some practices release records in a couple of weeks, while others take a month or more. We follow up regularly and keep you updated on progress so you're never left guessing.
Yes. We need a signed consent form or a solicitor's letter of authority before we can approach any practice or trust. Without it, the holder has no legal basis to release your file to us.
That's rare, but if it happens we'll tell you why and suggest the next step, which might mean involving the Information Commissioner's Office or seeking legal advice.
Yes. If you've moved around, we can request records from several practices at once, though each one is treated as a separate request and may take a different length of time.
Our fee depends on how many providers are involved and whether the records are held on paper or digitally. We'll give you a clear quote before we start any work.
Either. We can deliver directly to you or to a solicitor or other representative you name. Just let us know at the start and we'll follow your instruction.
Services
Where adult or child social services hold records, we can request those files if you have the authority to release them to us.
We handle requests to mental health trusts with extra care, ensuring the right consent is in place and the records are delivered discreetly.
Whether the care was NHS or private, we approach the relevant trust or hospital department for inpatient and outpatient records.
After collection, we scan every page and can supply the records as a searchable PDF, a printed set, or both, depending on your needs.
We request full GP notes, including appointment histories, test results and referral letters, from practices across the UK.
We retrieve occupational health notes from employers or their providers, often needed in employment tribunal or personal injury claims.
